Patrick McCaffery attended West High School in Iowa City, Iowa. At 14, he was diagnosed with thyroid cancer. He recovered and led West to a record of 17-5 by averaging 25.1 points and 7.6 rebounds per game. He shot 58 percent from the field and 45 percent form 3-point range and was named first-team all-state in Class 4A. He left as the all-time leading scorer at West High.

In 2019, he received the Mary Ann Raymond Donnelly Fighting Spirit Award.

In 2022, he was named as Iowa's Big Ten Men’s Basketball Sportsmanship Award winner.

He is the son of head coach Fran McCaffery and brother of Connor McCaffery.
