Since taking over the program in 2010, the Hawkeyes have vaulted back to national prominence. Iowa has appeared in a postseason tournament six of the last seven seasons (3 NCAA, 3 NIT). He coached the Hawkeyes to three straight NCAA Tournament appearances (2014, 2015, 2016) -- something that had not been accomplished in 23 years -- and reached the NIT championship game in 2013 with a team that won 25 games. In 2021, his team earned a #2 seed in the NCAA tournament, and in the 2022 season, won the Big Ten Tournament after losing 2 NBA draft picks from the previous season. He failed to qualify for the NCAA tournament in his final 2 seasons, and was let go after the 2024-25 season.

He left Iowa after 15 seasons as the all-time winningest men’s basketball coach with 297 wins. He also led the Hawkeyes to seven NCAA Tournament appearances. Off the court, The McCaffery family has long been a champion for Coaches vs. Cancer and the American Cancer Society (ACS), including raising more than $5 million since he became Iowa's head coach in 2010. He was honored during the 2015 Final Four weekend with the Coaches vs. Cancer Champion Award. The McCafferys also helped spearhead the launch of a new Adolescent and Young Adult Cancer Program in Iowa City.

He was hired at Penn (his alma mater) in March 2025. As a player at Penn, he played three seasons after transferring in from Wake Forest. Nicknamed "White Magic," he led the Ivy League in assists (105) and steals (51) as a senior in 1981-82 and was voted recipient of the Bus MacDonald Award as the team's most inspirational player that season. Penn went 36-6 in Ivy League play across the three seasons that he wore the Red and Blue, winning three Ivy League titles, and playing in the NCAA Tournament his sophomore and senior seasons. He then made his coaching debut with the Quakers following his graduation, as assistant varsity coach and head sub-varsity coach. He went on to lead four schools to the NCAA Tournament, Iowa, Lehigh, North Carolina-Greensboro and Siena.

He also coached his sons at Iowa, Connor McCaffery and Patrick McCaffery.
